Delete Your CareQuest Go Account

CareQuest Go is made by Chase Medical Services, LLC. You can request deletion of your CareQuest Go account and associated data at any time.

How to request deletion

1. Email dev@chasemedicalcares.com from the email address associated with your CareQuest Go account.
2. Use the subject line "Delete my account". You may request deletion of your entire account, or of specific data (such as care logs or photos) without deleting your account — say which in the email.
3. We will verify the request came from your account's email address and complete deletion within 30 days, confirming by reply when done.

What is deleted

• Your account and sign-in credentials
• Care plans, care logs, and photos
• Care Circle memberships and invitations
• Progress data (XP, levels, quests)
• Subscription entitlement records held by CareQuest Go

What may be retained

• Minimal records of financial transactions, as required for tax, accounting, and legal compliance
• De-identified analytics that cannot be linked back to you

Subscription billing itself is managed by Apple or Google — cancel active subscriptions in your App Store or Google Play settings. Deleting your account does not automatically cancel a store subscription.
